We are required by applicable federal and state law to maintain the privacy of your health information.  We are also required to give you this Notice about our privacy practices, legal duties and your rights concerning your health information.  We reserve the right to make changes in our privacy practices at any time, within parameters of the applicable law.

 

We use and disclose health information about you for treatment, payment and healthcare operations.  We will not use your health information for marketing communications without your written authorization.

 

You may give us written authorization to disclose your health information to anyone for any purpose.  You may revoke this authorization, in writing, at any time.  In the event you become incapacitated we will only disclose health information that is directly relevant to the person’s involvement in your health care.  

 

We may use your health information when we are required to do so by law.

 

We may disclose your health information to appropriate authorities if we believe you are a victim of abuse, neglect, domestic violence or the possible victim of other crimes.  We may disclose your health information to the extent necessary to avert serious threat to your health or safety, or that of others.

 

We may disclose your health information to appropriate authorities regarding work related illness or injury.  

 

We may, under certain circumstances, disclose your health information to authorized military officials, federal officials, law enforcement officials and other officials involved in national security.

 

We may use or disclose your health information to provide you with appointment reminders.

 

We do not have to obtain consent in emergency treatment situations, when treatment is required by law, or when communication barriers exist.
